- Name of the Insured
2. Incident/Occurrence Details:
- Date and time of the incident
- Location of the incident
- Description of what happened
3. Involved Parties:
- Names and contact information of all third parties or vendors involved
- Details of any witnesses
4. Damage or Loss Description:
- Description of the damage or loss
- Estimated value of the loss or damage
